Which hormone values does a testosterone test measure?

Eight values that together give the whole picture. Total testosterone is the starting point, but by far the largest share of it is bound to SHBG and albumin. Free testosterone and the free androgen index show what is actually available to your muscles, your energy and your libido.

LH and FSH tell you where a low value comes from: the testes themselves, or the signalling from the brain. Prolactin and estradiol complete the picture. Elevated prolactin can lower your testosterone, and estradiol shows how much testosterone is converted into estrogen. Every value comes with a plain-language explanation in the app and has its own page in the biomarker library.

02 Threshold

When is your testosterone too low?

There is no national threshold. Four Dutch labs, four lower limits: for men up to 50 the lower limit for total testosterone runs from 6.1 to 9.0 nmol/L, and at the three labs that publish free testosterone from 165 to 210 pmol/L. The same value is low at one lab and normal at another. That is why the app shows your value against the reference ranges of the lab that ran your sample, and the total testosterone page puts the labs' tables side by side.

Guidelines only speak of a deficiency when a low value has been measured twice in the morning and there are symptoms to go with it. One low value is a reason to repeat, not a conclusion. Which symptoms those are is explained under low testosterone.

03 Practical

What time should you draw for a reliable testosterone value?

Testosterone peaks in the morning and drops over the day. So book your draw in the morning, preferably before 10:00 and fasted. Reference ranges and guidelines assume a morning measurement, which makes your value comparable with the lab and with your next test.

Slept badly or just been ill? That lowers your testosterone temporarily. Draw on an ordinary day, so you measure yourself and not your week.

What does each value in the testosterone test measure?

Eight hormone values, each with its own role in the picture of your testosterone, plus one optional add-on: Prostate health (PSA).

Total testosterone
Total testosterone adds up all the testosterone in your blood, the part bound to protein and the free part together. It is the hormone that carries muscle, bone, desire and energy, and one of the hormones that move most with how you live.
Free testosterone
Free testosterone is the part of your testosterone not bound to protein and therefore able to do its work, calculated from your total testosterone, your SHBG and your albumin. It is the number that fits complaints best when SHBG is abnormal.
FAI (free androgen index)
The free androgen index is a calculation: your total testosterone divided by your SHBG, the protein that binds testosterone. The result estimates how much of your testosterone is free and therefore works, and is above all in women with excess hair growth, acne or an irregular cycle a more usable number than the total alone.
SHBG
SHBG, in full sex hormone binding globulin, is the protein from your liver that holds testosterone and oestradiol in your blood and so decides how much of them stays active. It is also one of the first values to give away insulin resistance.
LH
LH, in full luteinising hormone, is the hormone from your pituitary that in women gives the starting signal for ovulation and in men sets the testes to making testosterone.
FSH
FSH, in full follicle-stimulating hormone, is the signal with which your brain sets the ovaries or the testes to work: low when they respond well, high when they respond less, and permanently high after the menopause.
Prolactin
Prolactin is the hormone from your pituitary that starts milk production after childbirth and outside that time acts as a brake on your sex hormones: a raised value suppresses LH and FSH, and with them the cycle, desire and testosterone.
Oestradiol
Oestradiol is the main oestrogen: in women the hormone that carries the first half of the cycle and keeps bone, vessels and mucous membranes healthy, in men a small amount that arises from testosterone.
PSA
PSA, in full prostate-specific antigen, is a protein made almost entirely by the prostate, a small amount of which reaches your blood; on your report it is also called tPSA or total PSA. It is specific to the prostate, not to prostate cancer.
