LDH is an enzyme that sits in almost all your cells and that is released as soon as cells break down somewhere: in your muscles after a hard training session, in your red blood cells with breakdown, in your liver with damage, or simply in the tube with a difficult draw. It is a broad alarm, not a pointer.

What is LDH?

Every cell in your body makes energy, and when there is briefly too little oxygen, it switches to an emergency route in which LDH, lactate dehydrogenase, converts pyruvate into lactate. That is why the enzyme sits in almost every tissue: muscles, heart, liver, kidneys, lungs, and in large amounts in your red blood cells. If cells break down somewhere, LDH leaks into your blood, and the value rises. That makes LDH a broad alarm that points at no organ. There are five variants of the enzyme that differ per tissue, and those can be determined separately, but what stands on an ordinary report is the sum. The meaning comes from the company: high AST and CK alongside is muscle, high bilirubin and many reticulocytes alongside is breakdown of red cells, high ALT alongside is the liver. And one practical point colours everything: a red blood cell contains roughly fifty times as much LDH as the blood plasma. If cells break in the tube, with a difficult draw, you mainly measure that. Why is LDH relevant?

For anyone who does sport, LDH is first of all a training marker. After a hard strength session, a long run or a race, LDH, CK and AST sit above the band for a few days, because muscle fibres break down and repair with exertion. That is not damage to worry about, it is the reason you get stronger. Anyone who knows that has blood drawn on a rest day; anyone who does not reads a liver work-up that leads nowhere. The second reason is breakdown of red blood cells. In haemolysis, from antibodies, an inherited cell shape or a faulty heart valve, LDH rises together with bilirubin and the reticulocytes, while haemoglobin falls and haptoglobin disappears. That is a picture LDH makes visible first, and in runners it happens in a small way with every footstrike. LDH also rises with a deficiency of vitamin B12 or folate, because the marrow then makes oversized cells that already break down in the marrow. The third reason is what LDH is not. It is used in some forms of cancer to follow the course, because fast-growing tissue releases a lot of LDH, and that makes people afraid of a raised value. But in someone without a diagnosis a raised LDH is almost never cancer and almost always muscle, draw or red cells. It is a measure of course beside a diagnosis that is already established, not a test to find one.

  • Whether 'haemolysis' is noted beside the result, and whether potassium is high too; then it was the tube
  • AST and CK, which go up with muscle damage
  • ALT and the other liver values, which point at the liver
  • Bilirubin, reticulocytes and haemoglobin, which show breakdown of red cells
  • When you last trained hard
  • Your own lab's range, because the upper limit differs and sometimes rises with age

For LDH the range differs more than for most values: the national table uses an upper limit of 225 U/l, the labs themselves usually sit around 250, and some let the limit rise with age. So a value that is called normal at your own lab can sit above the national value without anything being wrong. A lower limit has no meaning. More important than the band is how the draw went and what you did in the days before.

LDH high or low: what it means

A high LDH means cells have broken down somewhere, and the first question is whether that happened in the tube. If the draw was difficult, red cells have broken and you measure their contents; usually 'haemolysis' is then noted beside the result, and potassium is falsely high too. After that, in order of how often it occurs: muscle damage after exertion, liver damage, breakdown of red blood cells, and rarely tissue damage elsewhere, such as a heart attack, a pulmonary embolism or fast-growing tumour tissue. AST and CK point at muscle, ALT at the liver, bilirubin and reticulocytes at red cells. A low LDH means nothing; it is simply little cell breakdown, and nothing belongs with it. What you do about it: with a rise after training nothing, except have blood drawn on a rest day next time. With a difficult draw you have blood drawn again. With a picture of breakdown of red cells or liver damage, investigation of that belongs, and then the values beside it matter more than the LDH itself. Never read LDH alone, but always beside AST, CK, bilirubin, the reticulocytes and your haemoglobin. Have blood drawn on a rest day, and compare over time at the same lab, because the upper limits differ.

LDH rises with every form of cell breakdown, and the everyday reasons, a hard training session, a difficult draw, breakdown of red cells, are far more common. Fast-growing tumour tissue also releases LDH, and that is why it is used in some forms of cancer to follow the course, but then beside a diagnosis that is already established and together with many other abnormal values. A slightly raised LDH with an otherwise normal blood count is no reason to think of cancer.

Can the blood draw affect my LDH?

Yes, strongly. A red blood cell contains roughly fifty times as much LDH as the blood plasma, so if cells break with a difficult draw, a thin needle or a tube left standing too long, you measure their contents. Potassium falls into the same trap, because that too sits much higher in the red cell. If 'haemolysis' is noted beside the result or potassium is high too, it was the tube, and you have blood drawn again.

What does sport do to LDH?

After a hard strength session, a long run or a race, muscle fibres break down and repair, and in the process LDH, CK and AST leak into your blood for a few days. That is not damage to worry about, it is the reason you get stronger. Why is LDH listed among the liver values if it comes from everywhere?

Because it rises with liver damage and is often requested in the same panel. Strictly it is not a liver value: it belongs just as much to the blood count, where together with the reticulocytes and bilirubin it shows breakdown of red cells, and to the muscles, where together with CK it shows muscle damage. Read it in the company your result asks for.

Do I need to fast for an LDH test?

No. LDH does not move with food. What does count: have blood drawn on a rest day, at least two days after hard exertion, and let the draw flow smoothly, because broken red cells in the tube lift the value sharply. AST, bilirubin and the reticulocytes come from the same draw, and without those LDH cannot be read.
